Care@Home.
A nurse practitioner or physician assistant comes to your home to give you extra attention, coordinated with your Breakwater Primary Care provider. Home based physical therapy, nursing, and social services available so you can get a high level of care in your own home.
Advanced Illness Program.
All the benefits of the Care@Home Program with even more intense attention from specially trained nurses who excel at caring for people in stages of advanced illness and palliative care so that you and your family can receive the highest quality of care and support in the home.
Nurse Care Management
Everyone knows the health care system is complex to navigate. Our nurse care manager will help coordinate your care and make sure you are on the same page with your treating providers. If you are hospitalized, our nurse will contact you shortly after you get home, answer all your questions and communicate with your Breakwater Primary Care provider, easing your transition home so you can make a good recovery.
Clinical Pharmacy Support
Our clinical doctor of pharmacy will review your medications for drug interactions and side effects, and support your Breakwater Primary Care provider as a resource to help optimize your regimen to the fullest benefit of your health. The pharmacist can also help find the best medications at affordable rates.
Conversio - Breathe Better
Work with a respiratory therapist to learn all the ways to breathe better when you have asthma, COPD, or emphysema. Get your medicine in a cleverly designed hand held nebulizer, making it easier to get the medicines into your lungs to open up your airways. Best of all, get your breathing medications for FREE. Social Isolation Support.
Studies show that socially connected people live longer; in fact, social connection increases the odds of survival by 50%. Our social workers will introduce you to opportunities for social engagement, assist in getting connected with programs, and make monthly check-in calls to follow up and provide support, all in coordination with your Breakwater Primary Care provider.
Heart Health Program
Partnering with the Cardiovascular Institute of New England, this program helps to empower patients to change heart failure to heart health, by quickly getting patients on the right medicines in therapeutic doses, provide health coaching including dietary sodium and volume management, same day appointments with a cardiologist for urgent needs, and timely access to important treatments like IV Lasix.
Remote Patient Monitoring
Have your blood pressure, weight, and oxygen levels monitored at home, with readings sent to our nurse care manager, who will track progress and notify your Breakwater Primary Care provider with any concerns.
